To be eligible for services, students must be currently enrolled in classes and have paid their health fees.
To request an appointment for an initial 30 minute phone, video or in-person screening, please fill out our online "Request Initial Screening" form. You will be prompted to enter your SMC student email user name and password in order to access the form. After the form is completed, you will be contacted via email with the date and time of your scheduled appointment. Please note that in-person appointments are limited and offered only on Mondays through Thursdays.
Before meeting with your assigned counselor, you will be asked to fill out a brief questionnaire and sign a consent form for services. These additional forms will be emailed to you on the day of your scheduled appointment.
