The following forms must be completed by students interested in participating in the Applied and Service Learning Program at Santa Monica College.
Student Alternative Placement Request Form - The Applied and Service Learning Program is open to students who are interested in completing their Applied and Service Learning requirement at a placement site, not already approved for their class. However, the program must ensure that the placement site meets the requirements of the Applied and Service Learning Program as well as serves the purpose of a student's class and course material. As such, students must submit the Student Alternative Placement Request form and receive approval if they are interested in completing their Applied and Service Learning requirement at an organization not already approved for their class.

Student Application and Agreement Form - The Student Application and Agreement Form is the official document used by the Applied and Service Learning Program to identify students, instructors, and community partners who are participating in the Applied and Service Learning Program at SMC. The form also includes a set of expectations the Applied and Service Learning Program requires all students to follow while participating in the Applied and Learning Program at SMC. This form must be completed and turned into the Career Services Center before a student may begin their service.

Student Time Log - The Time Log is used to assist the student in tracking hours completed at their assigned organization. It is the student's responsibility to record, monitor, and complete the minimum number of required hours as determined by his/her faculty and the community partner. The Time Log must be completed and signed by the student and community partner site supervisor. The form must be turned into the Career Services Center by the specified deadline provided to a class.

Student Evaluation - The Evaluation allows students to share their thoughts about their applied and service learning experience. It is also used by the Applied and Service Learning Program to assess the program. Student evaluations will only be emailed to SMC student email addresses. The form must be completed by the specified deadline provided to a class.
Student Application and Agreement Form - One Day Event - The Student Application and Agreement Form - One Day Event form is the official document used by the Applied and Service Learning Program to identify students, instructors, and community partners who are participating in events that only require student attendance at a specific event (i.e. A beach cleanup organized on a specific date and time). The form also includes a set of expectations the Applied and Service Learning Program requires all students to follow while participating in the Applied and Service Learning Program at SMC. This form must be completed and turned into the Career Services Center before a student may begin their service.

Minor Consent Form - Any student under the age of 18 years old who is enrolled in a course that includes an applied and service learning experience must submit the Minor Consent form to the Career Services Center. The form must be signed by the student's parent/legal guardian
