The following forms must be completed by local organizations wishing to partner with the Applied and Service Learning Program at Santa Monica College.
Community Partner Application and Agreement Form – The Community Partner Application and Agreement Form is the official document used by the Applied and Service Learning Program to identify organizations interested in participating in the Applied and Service Learning Program at SMC. The form also includes a set of expectations the Applied and Service Learning Program requires all organizations to follow while participating in the Applied and Service Learning Program at SMC. This form must be on approved and on file with the Applied and Service Learning Program, before any organization is added to the Community Partner Database as well as before any organization can begin hosting SMC applied and service learning students.

Community Partner Request Form – The Community Partner Request form helps the Applied and Service Learning Program learn about an organization, the community the organization serves, any participant requirements as well as opportunities for students to be involved. Furthermore, once an organization has been added to the Community Partner Database, the Applied and Service Learning Program uses the information on the Community Partner Request form to recruit applied learning students and to provide students and faculty with preliminary background information about an organization. This form must be on file with the Applied and Service Learning Program before any organization is added to the Community Partner Database as well as before any organization can begin hosting SMC applied and service learning students.
