The college wants to provide a safe and healthy place for the SMC community.
SMC follows the public health guidance and legal requirements for COVID-19.
- According to the Centers for Disease Control and Prevention (CDC), the California Department of Public Health, and the Los Angeles County Public Health Officer, COVID-19 continues to pose a risk, especially to individuals who are not fully vaccinated, and certain safety measures are needed to protect against COVID-19 cases and deaths.
- Vaccination is the most effective way to prevent transmission and limit COVID-19 hospitalizations and deaths.
- It’s important for the college to have information about vaccination rates among its community to make well-informed public health decisions about in-person activity on campus, setting testing frequency, and how to keep the campus community safe from COVID-19.
Exemptions and deferrals from COVID-19 vaccination requirements may be requested.
Securely upload your proof of vaccination or requests for exception/deferral at Vaccines.
SMC accepts vaccines authorized in the United States by the FDA (Pfizer-BioNTech, Moderna, and Johnson and Johnson (J&J)/Janssen COVID-19 vaccines) or those authorized by the World Health Organization (e.g., AstraZeneca/Oxford and Sinopharm).
All international immunization documents must be translated into English before submitting.
Contact the site where you received your vaccine. They may be able to provide you with proof of vaccination. See Vaccines for details on how to obtain your vaccination records if you lost your CDC vaccination card.
You can also contact your state’s health department. Some states have registries that include adult vaccines.
If you received separate vaccine cards for each dose of a multi-dose vaccine (e.g., Pfizer, Moderna), upload a photo or scanned copy of the front of each card.
If you made an error, you can update your record at any time. Just submit a new form at Vaccines.
Students must submit proof of vaccination if they intend to enroll in on-ground classes well in advance of their enrollment appointment or of the date when they intend to enroll if their appointment has passed. If you have only had once shot, wait until you have your second shot and then upload your full vaccination record. Partial vaccinations are not accepted by SMC.
After you complete the upload process, please wait 10-14 days for the review process. If there are any issues with your proof of vaccination, you will receive an email. Make sure the email address vaccinations@smc.edu is not in your spam.
You do not need to check the status of your upload. You will receive an email if there is a concern.
If you wish to check your status, log into MyForms to determine the status of your submission. Look for and open the form submitted.
Your vaccination submission will be reviewed within 10-14 days.
You will be notified by SMC email if your proof of vaccination was accepted, or it an exception/deferral was approved.

The portal displays best on a desktop or laptop computer. It is recommended you use the portal on a computer instead of a mobile device.
The form submission portal is HIPAA and FERPA compliant.
The information you provide will be used to prevent and respond to the presence of COVID-19 on campus and to comply with federal, local, and College public health guidelines. The information will be maintained in accordance with all applicable laws and public health regulations.
You are no longer required to participate in the ongoing testing program if you satisfy the following criteria:
- It has been two weeks since your final vaccine dose,
- You received an FDA or World Health Organization authorized vaccine, and
- Your proof of vaccination has been accepted and is reflected on your Corsair Connect portal as “OK – Fully Vaccinated.”
